after the concern is identified. Parental
consent is needed before doing an evaluation. |

The family's Service Coordinator makes sure the
evaluation and services are coordinated in a way
that meets the family's needs and priorities. |
An evaluation of the child's development in all
areas occurs.

If a delay is confirmed, offer to be part of the
family's service team and make sure your
services are written on the IFSP. |
