Making A Referral to "Early On"
 
A concern could be a physical or medical condition or a delay in development. Contact with a child occurs and a concern is identified Gather enough information (through observation, conversations or a screening tool) to determine whether a referral for an evaluation is needed. Talk to the family before making an Early On referral.
A referral must be made within 2 working days after the concern is identified. Parental consent is needed before doing an evaluation. A referral is made to Early On for an evaluation
The family's Service Coordinator makes sure the evaluation and services are coordinated in a way that meets the family's needs and priorities. An evaluation of the child's development in all areas occurs. If a delay is confirmed, offer to be part of the family's service team and make sure your services are written on the IFSP.

Information Early On needs from you when making a referral:

 

  • Name of child
  • Child's birth date
  • Child's sex
  • Child's resident school district
  • Child's address
  • Parent/guardian name
  • Parent/guardian address (if different from child)
  • Parent/guardian phone number
  • Person/agency making referral
  • Statement of concern by referral source
